Care Coordination of Older Adults: Process, Outcomes, and Cost



Description: Develop care coordination ontology that describes the activities used by nurses to coordinate care among patients. Then using Natural Language Processing techniques aided with the ontology extract activities from nursing narrative notes to build intervention profiles, based on which care coordination dose is computed for every patient.
